The “Bankruptcy Estate” (Ellwood Medical Center, LLC d/b/a Ellwood City Medical Center [Ellwood Medical Center Real Estate, LLC; Ellwood Medical Center Operations, LLC] and its parent, Americore Holdings, LLC [Americore Health, LLC; Americore Health Enterprises, LLC]) has announced that patients of the Ellwood City Medical Center have until April 8, 2022 to request medical records.

An “Authorization for Release of Health Information” form will be emailed to the patient for their completion and returned by email for processing. If the patient does not have access to email, the form may be completed over the phone and signed at medical record pickup.
If any medical records of any patient of Ellwood City Medical Center are not requested by the information provided within 30 days of this notice, April 8, 2022, the records will be destroyed.
